CLIENT: NORTHBAY HEALTHCARE
NorthBay Healthcare, an integrated delivery network based in California asked us to assess readiness and plan implementation of Patient Centered Medical Homes for its four primary care practice sites.
Case Study 02: Assisting Improvement Collaborative
Client: Quality Partners of Rhode Island
Quality Partners of Rhode Island (QPRI) needed help providing support to medical home practices participating in an initiative to reduce unnecessary hospital readmissions and emergency department visits. An additional requirement was the ability to train and mentor their practice coaches, and provide content for physician and staff learning sessions.
